Family PACT is California’s innovative approach to provide comprehensive family planning services to eligible low income (under 200% federal poverty level) men and women. Services include comprehensive education, assistance, and services relating to family planning.
The Child Health and Disability Prevention (CHDP) is a preventive program that provides periodic health assessments and services to low income children and youth in California. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments are provided onsite at our community clinic; with managed care plans, and some local school districts with programs such as:
- Children in Head Start and State Preschool programs.
- Children in Foster Care.
